This support is funded through Capacity Building – Increased Social and Community Participation and focuses on mentoring, peer support, and practical skill development.

What Is Assistance in Coordinating or Managing Life Stages, Transitions and Supports Under the NDIS?
Life Transition Planning supports participants to manage important changes and responsibilities in their lives.
According to the NDIS Pricing Arrangements, this support includes mentoring, peer support, and individual skill development to help participants build practical life skills within their home and community.
It focuses on strengthening independence, social connection, and decision-making.
These supports may be delivered:
Individually, or
In group settings where appropriate
They sit under Capacity Building – Increased Social and Community Participation and are designed to help participants build confidence, connection, and practical life skills within their community.
What Can Be Claimed Under Assistance in Coordinating or Managing Life Stages, Transitions and Supports?
This line item can be used for structured mentoring and support in areas such as:
✔ Managing Appointments and Services
Attending medical or therapy appointments
Learning to schedule and manage bookings
Communicating with service providers
✔ Financial and Daily Management
Support with bill paying and budgeting routines
Understanding basic finances
Managing subscriptions and expenses
✔ Community and Social Participation
Attending social activities
Building friendships
Maintaining contact with family and peers
Joining community groups

What This Support Does Not Cover
Life Transition Planning does not include:
Clinical therapy or counselling
Psychological treatment
Financial planning or legal advice
Case management (unless funded separately)
Allied health assessments
This support focuses on practical mentoring and life skills, not professional treatment services.
Additional Claims Allowed
When providing Life Transition Planning supports, providers may also claim for:
Non-Face-to-Face Support (planning, coordination, and reporting)
Provider Travel (time)
Short Notice Cancellations (7 days’ notice)
Provider Travel – Non-Labour Costs (Code: 09_799_0106_6_3)
Activity-Based Transport – Capacity Building (Code: 09_590_0106_6_3)
All claims must be directly connected to the participant’s supports.
