I would like to book ALL Term 3 Sessions

I would like to discuss extra support (outside the scheduled time of 10am - 3pm)

I consent to the child listed participating in the Teens Social Club as outlined in my date selections*

I Understand that $52.00/hr will be charged from Core Supports (1:2 Saturday Rate + Centre Capital Costs)*

I understand that if I require transport to and from the Hub or have requested longer hours than the group session, I will be provided a quote for extra supports and travel*

I understand that I will be provided a Service Agreement for the supports listed above*
